Consultation on Arsenic & Health
Dr. D.N. Guha Mazumder, Director, DNGM Research Foundation visited Cambodia between 1st to 11th October 2006, being invited by WHO, South East Pacific Region, to support and assist the Ministry of Health and Ministry of Rural Development, Govt. of Cambodia, on detection, confirmation and management of arsenicosis, training of doctors and preparation of guidelines for case detection.

Arsenic Problem in Cambodia

Up to December 2005 about 15673 wells were tested for arsenic by Field Test kits in 7 provinces of Cambodia. The results showed that 2924 wells (18.5%) of the total wells tested proved to have arsenic concentrations above the national limit of 50 ppb. The arsenic contamination of ground water did not occur evenly in Cambodia. The most highly affected province was Kandal. Forty seven percent of the wells tested in this province have arsenic level >50 ppb.

Drinking Water problem in Prek Reusey village

The village chief of Prek Reusey village, Kampong Kong Commune, Koh Than district of Kandal province reported that during the last 2-3 years 3 young people died who had skin problems. Most villagers who have skin problems in the Prek Reusey village consume water from tube wells all year round. There are 75 tube wells in Prek Reusey village, including community wells, private wells, and school well and Pagoda. The arsenic level in 70 tube wells varied between100-500 ppb.

 
Clinical Study
Field visits were carried out in the first week of October 2006 by Dr. Guha Mazumder, the WHO advisor, accompanied with team of doctors, technicians, WHO Environmental Engineer, UNICEF functionaries and officials of MRD in the Prek Russey village.
Ninety-seven subjects belonging to 40 families were examined. Most of these people had evidences of chronic arsenic exposure from reports of testing of water samples (57) and of hair and/or nail studied in 93 subjects. Seventy cases (72.16%) were diagnosed to be suffering from arsenicosis. One case was found to be suffering from suspected skin cancer
Samples of water could be collected from present and past water sources from households of 57 participants on the second day of field visit. Very high level of arsenic contamination was found in the wells of Prek Russey village. Highest value of arsenic exposure, 1286 ppb, was found to occur in 29 subjects studied, the lowest range being 0.148 ppb. Duration of arsenic exposure varied from 5-10 years.
Arsenic level in nail or hair of 93 participants showed value above normal limit suggesting that nearly all the participants who attended the health camp in the Prek Reussy village had chronic exposure to arsenic. The minimum and maximum arsenic values detected in the nails were 1.06 and 69.48 ppm and in hair between O.92 and 10.9 ppm respectively.
Training of Doctors and health providers:

Two days workshop for training on arsenicosis case detection and management was held in a hospital at Phnom Penh being organized by MOH. In all 79 participants attended the workshop. The following categories of personnel were present in the meeting.

    • Directors of Provincial Health Department
    • Directors of Provincial Rural Development Department
    • Directors of Referral Hospitals
    • Technical Bureau Chiefs
    • Officials of Health Promotion Unit (Clinical Bureau)
    • Officials in charge of Continuing Education in the Province.
 
Technical guidelines provided
Technical guidelines on the detection and management of arsenicosis cases in Cambodia in accordance to WHO protocol have been prepared and submitted to WHO-Cambodia.
 
Mitigation Guidelines submitted

Suggested integrated guidelines for mitigation of arsenicosis in Cambodia have been submitted.
